Case Management Services (Home Care & Home Health Care)

 

CAS’ medical team serves as the go-between among doctors and their patients who have long-term health problems. They are primarily responsible for monitoring patients’ treatment and insurance coverage plans to ensure that their needs are entirely met.

Activities include:

• Case management to develop a transition plan
• Attendant services to assist with activities of daily living and support to identify financial assistance
• Adaptive equipment loan services
• General assistance in finding affordable and accessible housing if necessary
• Peer support and advocacy
• Information and referrals to resources and Independent Living Skills training and guidance
